Health Management Programs

McLaren Health Plan Community - Group / Individual (MHP Community - Group / Individual ) is committed to a proactive approach to management of the membership. Two areas that are focused upon to promote the well being of each member are disease management and preventive health management. Early assessment of the membership for chronic disease, special needs and preventive education is essential to improving outcomes, changing life styles, controlling cost and increasing membership satisfaction.

If a MHP Community - Group / Individual member is identified as having a condition such as Asthma, Diabetes, Depression, or Pregnancy, the case management department initiates a disease management process. Eligible members are identified on an ongoing basis through claims and encounter data, pharmacy reports, in-patient hospitalizations and through direct referral from the primary care physician. The REFERRAL TO CASE MANAGEMENT form should be used if you identify a member who would benefit from one of these programs, or call our nurses at (888) 327-0671 to enroll a member who needs assistance. Disease management provides a broad focus for the member while addressing a wide range of needs and allowing for more control of the member’s condition.
